I’m listing these acceptable or better, which is my honest floor: they show typical cosmetic wear - a scuff, a slightly worn label, the normal life of a 35-year-old cartridge - but they work, and the photo is representative of that bucket, so cosmetic details vary a little copy to copy. More than a Tetris knockoff - the color-matching pioneer

Released in 1990 out of Nintendo R&D1 (Gunpei Yokoi’s group), Dr. Mario took the falling-block puzzle idea somewhere new. The two-player versus mode is the beating heart of this game, and it has a real, organized 1v1 tournament scene: the Dr. Mario Championship, played on original NES hardware and CRTs, round-robin qualifying into a single-elimination bracket with best-of-5 finals at the punishing Level 14 “Hi” speed - original controllers only. It runs an annual championship plus regional qualifiers at the big retro expos (Midwest Gaming Classic, Portland Retro Gaming Expo), often sharing a hall with the Classic Tetris crowd. It’s grassroots - think the Dr. Mario answer to CTWC, a fraction the size but the same real-hardware, no-emulator spirit. This cart is the actual thing that scene competes on.

Direct ancestor of Dr. Mario World

That 1990 capsule mechanic ran straight forward into Nintendo’s 2019 mobile game Dr. Mario World - same match-the-capsule DNA, three decades later (RIP, it was shut down in 2021). In between came a whole prescription pad of sequels: Tetris & Dr. Mario, Dr. Mario 64, Dr. Luigi, and it’s playable today on Nintendo Switch Online.
